This application form is designed for existing Creative Spaces licensees. Please note that you are applying for a new licence and this is not considered a renewal or extension.
Below is a brief outline of the attachments and uploads needed to complete this form:
- Your current CV
- Any collateral you created that acknowledged Creative Spaces program or included the Creative Spaces logo
- Examples of what you've being working on in your studio over the last 12 months (upload a file or provide a link to a webpage)
- Public Liability Insurance Certificate of Currency
- Photo evidence of Test and Tag
